Job Purpose
The Program Nurse LPN will be clinically competent while delivering care to patients, utilizing the nursing process of assessment, planning, intervention, implementation and evaluation. The Program Nurse will effectively interact with patients, significant others and other health team members while maintaining all standards of professional nursing. The Program Nurse provides quality administrative and clerical services for program staff. They provide transportation to patients in a provided vehicle, as applicable. The Program Nurse responsibilities include assisting with insurance verification and billing procedures, providing clerical support to team members, and ensuring transportation is safely provided to patients. The Program Nurse contributes to positive team dynamics and excellent customer service.

  • The Program Nurse assesses and monitors clients’ health status and physical care needs at admission and while clients are in the program.
  • Completes an admission nursing assessment for every client according to program standard.
  • Assist patients with completion of consents and patient admission process as directed
  • Assist patients in performing the sit to stand test as directed
  • Complete the daily nursing checklist form as directed
  • Completes monthly falls assessment and implements fall prevention protocols as indicated.
  • Regularly obtains and documents patients vital signs and weights according to program standard.
  • Communicates relevant clinical and other client care information to program director, medical director and treatment team members as necessary and appropriate.
  • Monitors clients on an ongoing basis to identify health needs, problems, and/or symptoms.
  • Coordinate meals and snacks for patients
  • Assists patients with personal care needs (bathroom, etc) as needed or requested.
  • Coordinates and documents patients care in cases of medical emergency, providing a report to the Emergency Department or other appropriate resource.
  • -Assists patients on and off van as needed.
    - Assists with obtaining needed information from primary care providers.
  • Ensure completion of the Census report as directed
  • Complete multidisciplinary progress notes for documentation of absences or patient encounters as they occur
  • Complete medical records including filing and form tracking
  • Participate in treatment team meetings and coordination of care
  • Responsible for inventory of supplies, cleanliness, group room setup
  • Responsible for refrigerator logs and necessary action steps for temperature variances

  • Compliance: Ensure the program operates within all regulations
  • Comply with 25-point check list
  • Comply with all regulations
  • Completes client billing procedures according to hospital policy.
  • Ensure patient insurance verification and complete monthly insurance verification forms as directed
  • Assists with ensuring regulatory compliance.
  • Implements and documents nursing interventions that are part of the interdisciplinary treatment plan.
  • Participates in the development of a therapeutic alliance between the interdisciplinary treatment team patients and their families.
  • Adheres to safety and infection control guidelines.
  • Participates in the completion and maintenance of patient’s medical records, including recording of physician’s orders.
  • Participates in quality improvement activities as requested.
  • Coordinates patient transportation.

  • Provides health education for patients, families and community groups as needed.
  • Provides education to the community on mental illness, or related health topics as assigned by the Program Director.
  • Provides families and family caregivers with education on health-related topics.
  • Participate in customer service training as it relates to community integration.
  • Participate in community analysis
  • Keep up to date community education contacts as directed
  • Participate in cohesive team environment to ensure program success

  • Qualifications
  • Education: Graduation from a state approved school of nursing.
  • Certification: Licensed Practical Nurse required.
  • Experience: Psychiatric Nursing experience preferred.
  • -Experience in providing care to older adults in hospital or nursing facility environment preferred.
  • Experience performing secretarial or clerical duties, working with computers and electronic medical records
  • Good driving record, comfort with transportation and driving a van
  • Experience transporting patients, preferred but not required
  • Knowledge of community resources for older adults, familiarity with medical terminology and medical team roles functioning with other hospital departments
  • Other skills:
  • Patient and family teaching; teaching/learning materials development; problem solving; complaint resolution; ability to make presentations to outside groups, good problem solver, results oriented.

    Physical Requirements
    While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. This position required intermittent physical activity, including standing, walking, bending, kneeling, stooping and crouching as well as lifting, driving and supporting patients.
